A recent investigation by the Ministry of Labour’s Occupational Safety and Health (OSH) Department has concluded that a 19-year-old excavator operator, Ezekiel Benjamin, died while operating in an unstable working environment. The incident occurred on November 12, 2024, at the Kara Kara area in Mackenzie, Linden, where Benjamin was employed by Dindyal Sookram and Daughters General Contractor for sub-contract work.
Benjamin was tasked with operating an excavator to construct a drainage system intended to channel water from the surrounding area to Kara Kara Creek. However, the excavator began sinking into the swampy terrain and became submerged, trapping Benjamin inside. Despite efforts by nearby operators from BOSAI Minerals Group to rescue him, Benjamin was pronounced dead upon arrival at the Mackenzie Hospital Complex.
The investigation revealed several safety breaches, including non-compliance with sections of the Occupational Safety and Health Act. Key findings included the absence of an Occupational Safety and Health Policy, lack of a safety and health representative, and inadequate supervision. The employer was found to have failed to provide a safe working environment, as required by law.
The Ministry of Labour has expressed sympathy to Benjamin’s family and emphasized the importance of good safety and health practices in the workplace to prevent such incidents. Recommendations have been made to ensure compliance with safety regulations, and follow-up actions are being taken to address these concerns.
